Parental Agreement.

1. Introduction.

In order for the Club to be ‘Covid compliant’ the FA Guidelines and Club Health and Safety Recommendations must be strictly adhered to. Where doubt exists as to the course of action that should be followed then the Club aim to follow a ‘Safety First’ approach. This approach should allow the ‘chain of responsibility’ to remain intact and hence the Club can operate to a standard of safety that satisfies Health and Safety (H&S), FA Safeguarding and Insurance requirements. As parents or carers and in order to satisfy the Club’s safeguarding requirements we must obtain initial permission for players to attend both training and matches.

The Club must also record individual attendance at each match and training session. This is an FA requirement to support Track and Trace. By giving initial permission to attend Club training and matches you also agree to use TeamApp to record attendance at each Training Session or Match.
Other responsibilities are laid out in the FA and Club H&S Guidance that we ask you to read.

2. Agreement.

I have read the Club Health and Safety Guidance and will adhere to the guidelines.
I have read References A,B and C below and will support the Track and Trace requirements for each training session and match through the use of TeamApp.
